  • Everything included in our Standard Concierge Medicine Package, plus enhanced services

  • Dr. Eller travels to you at your home or office  for medical visits,  up to 12 times a year (within 30 miles of the Elevate Health office)

  • Enhanced care coordination with specialists and other healthcare providers

  • Coordination of care with out-of-state primary care providers, if applicable

  • Mobile lab services performed at your home or office, coordinated through Concierge Lab Services. (4 per year included)
