Only need a few visits per year
Have limited or straightforward health needs
Prefer to manage their own referrals and imaging
You’ll receive the same comprehensive, unhurried care that Elevate is known for, with the flexibility to pay only for the time you use.
Medical Visits
Address acute or chronic health concerns directly with Dr. Linda Eller, DO. Choose the appointment length you need—from 15 minutes to 90 minutes—and take control of your visit.

Weight loss Visits
Weight loss is a personal journey. Dr. Eller works with you to design a tailored approach that may include:
- Nutrition and lifestyle recommendations
- Exercise guidance
- Prescription oral or injectable weight loss medications (if appropriate)

Boost your energy, metabolism, or hair health with personalized vitamin shots. Options are discussed during your visit to fit your health goals.

A comprehensive wellness experience that includes:
- 15-minute phone consultation to customize your exam
- Personalized health risk assessment
- Comprehensive lab panel orders
- Coordination of care for in-home lab draw
- Imaging orders
- EKG
- Full interpretation of all labs and imaging
- 1-hour physical exam
- A detailed, integrative treatment plan you can understand and follow
Pre-payment required to order labs and imaging so they can be completed before your exam.

Preoperative Clearance
Includes everything you need for safe surgical preparation:
- Pre-op physical exam and clearance by a physician
- Coordination of care for in-home lab draw
- EKG
- Interpretation of labs and imaging
- Official medical clearance letter
- Coordination with your surgical team
- Nutritional recommendations to optimize healing

Trigger Point Injections
Effective relief for muscle knots and chronic pain. These injections target specific points that remain tense and painful, even when the surrounding muscle is relaxed.
