With the Elite Package, Dr. Eller travels to you, providing in-home or in-office visits for you and your family (within a 30-mile radius of her office), with up to 12 visits per year. This package also includes enhanced care coordination with specialists and other healthcare providers, ensuring seamless communication across your care team.
For patients who maintain a primary care provider in another state, Dr. Eller offers ongoing coordination of care with your out-of-state PCP, helping maintain continuity no matter where you live or travel.
To further simplify preventive care, the Elite Package includes four complimentary mobile phlebotomy lab draws per year, coordinated through Concierge Lab Services, performed in the comfort of your home or office.

No. Elevate Health & Aesthetics does not bill insurance. This allows us to provide truly personalized, unhurried care without restrictions. You may use insurance for labs, imaging, medications, or referrals to outside specialists when applicable.
Yes. While we don’t bill insurance for office visits or services, you can still use insurance for most labs, imaging, prescriptions, and specialist visits.
If you don’t need full membership, we offer an hourly fee option for non-members. This includes annual exams, sick visits, preoperative evaluations, and more.
Yes! Functional medicine is integrated into your care as needed—especially for uncovering root causes of chronic symptoms, hormone imbalances, fatigue, gut issues, and more.
We provide direct referrals and help coordinate care with trusted specialists. We can also schedule imaging and follow-up appointments for you.
